Description

Licensed Practical Nurse position in the Primary Care department in Modena, NY. 40 hours per week, 8‑hour shifts Monday through Friday; 8:00 AM–5:30 PM. Hours vary.

Summary

Performs tasks and responsibilities within case finding, health teaching, counseling, and supportive care under the direction of a registered nurse or licensed health care provider.

Responsibilities

• Greet patients in the waiting area, escort patients to clinical areas, and escort patients to checkout.
• Answer patient questions about the day's appointments, treatments, and follow‑up.
• Handle telephone call‑backs and telephone care as directed by the physician/provider and RN.
• Provide patient counseling and education as directed by the provider (verbal, brochures, handouts).
• Focus on patient needs while maintaining an organized and efficient workflow as established by Nuvance Health's policies and standards. Responsible for HPN and NYSIIS immunization registration, reprinting, and account updates.
• Ensure that patients have follow‑up testing and appointments with physicians.
• Prepare the medical record in advance of the appointment.
• Assist patients with completing patient summaries, medication/allergy reconciliation at every office visit, history forms, and obtaining appropriate treatment consents.
• Prepare the patient and exam room; chaperone and assist the physician/provider with examinations, procedures, and treatments if needed.
• Administer and document injections; dispense and document medications as directed by the physician/provider.
• Label and handle labs, paps, cultures, biopsies, and other specimens; prepare laboratory and imaging requisitions.
• Assist with coordination of further patient care and follow‑up, and carry out tasks as directed by the RN and/or physician/provider. This may include communication with patients, hospitals, labs, imaging centers, pharmacies, physician offices, and insurance plans.
• Clean and prepare the exam room for the next encounter.
• Perform other duties as required.

Required

• Basic Life Support (BLS) certification within 30 days of starting.
• Licensed Practical Nurse licensure.
• Basic medical knowledge and terminology.
• Computer skills/data entry.
• Skilled in obtaining accurate patient vitals.
• Vaccine administration and medication preparation.

Preferred

• Minimum experience: one year.
• Proficiency in multiple languages.
• Phlebotomy.
• EKG.

Education Required

• High School graduate or equivalent.
• Graduate of an accredited LPN program.

Working Conditions
