Care Manager – Emergency Department (8-Week Contract, Hybrid)

New York, NY | Hybrid | 5x8 Rotating Shifts (8 hrs)

Travel: $2,474 weekly gross ($17/hr W2 + $1,897 weekly per diem)

Local: $60/hr on W2

We’re looking for an experienced Registered Nurse with strong care management and clinical coordination skills to join our team for an 8-week contract (with potential extension). This hybrid role combines clinical expertise with care planning and patient advocacy—helping members navigate the healthcare system and improving outcomes across the continuum of care.

What You’ll Do

• Review and evaluate assessments and UAS information for members in MLTC and MAP programs.
• Collaborate with primary care providers to align on care needs and identify risks.
• Develop and monitor individualized care plans, including disaster and prevention plans.
• Coordinate services across inpatient, outpatient, and community settings.
• Identify opportunities to reduce unnecessary hospitalizations or ED visits.
• Conduct regular check-ins (phone or in-person) and maintain timely documentation.
• Work closely with Utilization Management, Quality, and Customer Service teams.
• Participate in team meetings and the department’s on-call rotation.

What We’re Looking For

• Active RN license in the State of New York (required).
• BSN required; Master’s degree preferred.
• Five+ years of clinical nursing experience (leadership experience a plus).
• Background in care coordination, utilization review, and discharge planning .
• Strong computer literacy and documentation skills.
• PRI and Screen certification (or obtained within three months of hire).
• Bilingual Spanish and/or Case Management Certification preferred.
