🚀 Health Guide (NP) - Part-time

A healthcare organization in Virginia is actively seeking a licensed Nurse Practitioner (NP) to join their staff as a new Health Guide on a Part-time basis. In this role, the Health Guide (NP) will assess both behavioral and physical health conditions through a thorough history and physical examination
Responsibilities:

• Capturing conditions not yet diagnosed, including by ordering additional studies as indicated
• Organizing the information in a manner that supports successful disability applications
• Conducting visits in the field or in an individual's home when needed
• Making individualized recommendations and/or referrals for programs/care
• Training and advising Guides and Community Resources Guides on clinical issues and treatments
• Supporting Guide Team on care coordination
• Leading care management
• Performing medication reconciliation post-discharge and as needed
• Communicating and coordinating with individuals' care providers, primarily at CMHCs, FQHCs, and hospitals

Qualifications

• Active VA Nurse Practitioner (NP) license
• 2+ years of Clinical Care experience as a Nurse Practitioner
• Experience working with and managing a population with challenges such as behavioral health and/or SUD conditions
• Experience developing and delivering educational programs to nurses and affiliated medical staff
• Care management and coordination experience
• Intermediate computer skills; familiarity with Microsoft Office Suite
