Job Description Summary
The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with policy, professional standards, and applicable regulatory entities. This is an entry-level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use.
Job Responsibilities
- Determines the suitability of individual patients for home care. Collects and organizes patient-specific information to determine suitability and prognosis with defined outcome goals.
- Designs, monitors, and evaluates patient-specific pharmacotherapy care plans for home care patients. Recommends modifications when appropriate and identifies monitoring parameters. Completes nutritional assessments and makes recommendations.
- Modifies care plans based on evaluation of monitoring data. Applies pharmacokinetic principles and assesses patient laboratory values.
- Provides medication-use education to Option Care staff, other professionals, home care patients and caregivers; uses effective counseling techniques.
- Ensures continuity of pharmaceutical care to and from the home and other patient-care settings. Prepares and dispenses medications following policies and procedures; calibrates equipment; ensures appropriate storage, labeling, safety, and quality assurance; verifies authorization to dispense and complies with laws and USP standards.
- Manages use, maintenance, and troubleshooting of medication administration equipment. Advises others on equipment use and maintenance; participates in incident management and reports discrepancies in inventory systems.
- Provides concise responses to drug information requests from health care providers and home care patients; participates in continuing education to maintain professional competency and complies with state CEU requirements.
- Manages investigational drug products per regulations and Option Care policies; prioritizes workload and ensures accuracy of work by pharmacy support personnel.
- Complies with accreditation, legal, regulatory and safety requirements; maintains patient and proprietary information confidentiality in line with legal and ethical guidelines.

Education and Experience Requirements
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following: (a) at least one year of experience in sterile IV admixture procedures, (b) knowledge of non-sterile compounding, immunizations and MTM, (c) completion of an ASHP-accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience, or (e) equivalent experience as approved by leadership.

- Experience providing customer service to internal and external customers and evaluating customer satisfaction.

Travel Requirements
Willing to travel up to 5% of the time for business purposes (within state).
Preferred Qualifications & Interests (PQIs)
- Completion of an ASHP-accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience.
- Experience in home care, hospital or other clean room setting.
Management Requirements
At least 1 year of indirect team management experience, including assisting in development, training and assignment of work/projects. May perform other duties as assigned.
