Case Manager

Location: 97 Crooke Ave, Brooklyn, NY 11226

Responsibilities

• Review and maintain tenant files and documentation.
• Conduct initial intake, periodic assessments and independent living plans.
• Assist tenants in accessing community resources and referrals.
• Monitor progress, provide progress notes and complete required reports.
• Participate in case conferences and liaison with external organizations.
• Provide additional case management duties as outlined (e.g., escort appointments, benefit application assistance).

Qualifications

• Bachelor’s degree and experience working with the Seriously and Persistently Mentally Ill population.
• Flexible hours may be needed occasionally to provide shift coverage until 8 pm.

Compensation

$40,000-$45,000 annually
