Key Attributes

  • Accountability for outcomes: the PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients. They influence patient outcomes by building trust and helping change behaviors.
  • Coaching for health: the PCP acts as a health coach, helping patients set health goals and providing follow‑up to improve outcomes.
  • Simplifying for action: the PCP simplifies and prioritizes appropriately so that behavior change is actionable for both patients and their care teams.

Essential Job Duties / Responsibilities

  • The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults.
  • The PCP takes full accountability for patient care and outcomes and seeks specialist consultation when needed while remaining responsible for detailed patient care.
  • The PCP engages with the hospitalist whenever one of their patients is hospitalized, regardless of hospitalist affiliation.
  • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
  • The PCP leads the care team—including care promoter, care facilitator, and care coordinator—for patients able to come to the office.
  • For patients unable to come to the office (hospital, SNF, LTC, or homebound), the PCP engages with the transitional care team and other resources available in the market.
  • The PCP plays an active role in center management and covers for other providers when needed, including recruiting patients and additional providers.
  • The PCP supervises, collaborates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the APP’s actions while employed with the company.
  • The PCP performs other duties as assigned and modified at the manager’s discretion.

  • Self‑care: The PCP maintains physical, mental, social, and spiritual well‑being to best serve patients.
  • Technology proficiency: The PCP is proficient in Microsoft Office Suite and other relevant software used in the company.
  • Travel willingness: The PCP travels locally as needed, nationwide for initial training (2 weeks), and occasionally regionally or nationally for recruiting or training.
  • Language fluency: The PCP is fluent in English (verbal and written); additional language fluency may be required depending on the center.
  • Independent judgment: The PCP consistently exercises independent judgment in clinical and administrative decisions.

Education and Experience Criteria

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (required).
  • Current, active State Medical License in the state(s) of practice (required).
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (preferred) with board eligibility required.
  • Maintenance of board certification through MOC, CME, or board exams as required.
  • Current DEA number for schedule II‑V controlled substances (required).
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross within the first 90 days of employment (required).

Pay Range

$214,700 – $306,714
