Responsibilities
Assess patients' conditions and develop individualized care plans centered on comfort and symptom management.
Administer medications and treatments to manage pain and other distressing symptoms.
Provide emotional support and education to patients and families, helping them understand the hospice journey.
Collaborate with physicians, social workers, chaplains, and other members of the care team.
Monitor patient progress and adjust care plans as needed.
Maintain accurate clinical documentation in accordance with hospice regulations and standards.
Offer compassionate presence and reassurance during the final stages of life.
Required Qualifications
Current RN license
BLS (other certifications as required by facility)
Two years of recent Hospice RN experience
Strong communication and adaptability skills
