Licensed Practical Nurse (LPN) - Home Health

What You’ll Do
- Provide skilled nursing care and support to patients in their home environment
- Coordinate care plans with the clinical team to ensure optimal outcomes
- Monitor patient health, administer medications, and educate patients and families on continued care
- Document visits accurately and in a timely manner
- Maintain a positive, professional, and compassionate attitude in every interaction
